To register, you will be required to complete a registration form and supply identification paperwork, preferably photo ID, i.e. driver license, utility bill (within last 3 months).
You may be offered an appointment to meet with the doctor. Please provide a printout of your regular medications.
Your full medical record may take a few weeks to reach us; therefore a short questionnaire will need to be completed. Acceptance is at the discretion of the doctor.
